Cardiology Care At Home service is a collaboration between Ninkatec and Sunrise General Medical for patients who need treatment for heart and other cardiovascular conditions such as heart failure and atrial fibrillation while being cared for at home. This solution would also be helpful for patients with mobility issues or medical conditions making trips to and from the clinic or hospital challenging.
Patients will be visited once or twice a day by a nurse or cardiologist who will then communicate with each other. Visits can be conducted once a month or even once every 3 to 6 months for more stable situations. If families need it, the plan may be customized to increase the frequency of these visits.
Patients are required for initial assessments to examine their condition based on heart rate, oxygen saturation, and severity of the heart disease. Eligible patients will have various monitoring systems to keep track of health with support from the cardiology specialist team.
With this home care service, patients can receive timely access to the attention needed for their heart or cardiovascular condition. This includes:

● Urosheath/urine output monitoring
● 24 hour BP monitoring
● Telehealth BP monitoring
● Cardiologist home visit
● Nursing visit
● Cardiology support at home
For telehealth BP monitoring, the patient has to monitor BP at regular intervals with his/her own BP monitor to give feedback to Ninkatec care team.
